PKV Application Checklist

• Valid passport or national ID card Must be valid for the duration of the policy start
• German residence registration (Anmeldung) Confirmation letter from the Einwohnermeldeamt
• EU Blue Card / work visa / residence permit Any permit granting you right to work in Germany
• Tax ID number (Steueridentifikationsnummer) Sent by post after Anmeldung — 11-digit number

• Employment contract (Arbeitsvertrag) Shows your employer, role and gross salary
• Last 3 months' payslips (Gehaltsabrechnung) Confirms current income for premium calculation
• Last year's income tax return (if available) Especially important for freelancers and self-employed
• Freelancers: Gewerbeschein or Freiberufler confirmation Proof of self-employment registration with Finanzamt

PKV underwriting requires a full health declaration. Inaccurate answers can lead to policy cancellation. Have the following ready.

• List of all current medications Generic name, dosage, and reason for use
• Medical history: diagnoses from last 5 years Include hospital stays, surgeries, chronic conditions
• Previous GKV/PKV membership confirmation Your current insurer can issue a Versicherungsnachweis
• Specialist letters or recent lab results (if applicable) Helps broker recommend the right insurer for your profile

• German bank account (IBAN) Premiums are collected by direct debit (SEPA Lastschrift)
• German mobile number or contact email For policy documents and communication from insurer
